I will be out of the office 7/12- 7/15

POLICIES:

* Each Session is tailored to the clients specific needs mixing relaxation and/or deep tissue work at a comfort level that the client dictates. Feedback is openly appreciated and necessary to tailor the treatment to the needs of the client.

* Massage Therapy For Health practices relaxation and therapeutic deep tissue massage. This is a non-sexual practice. If the therapist feels sexually harassed, or in danger at any time, the session will end immediately.

* Liliane Sklenarik is a licensed and certified massage therapist. She is a Professional member of the AMTA. She is nationally certified through NCBTMB. To retain these memberships, she is required to take accredited courses every 4 years. She is able to perform only under the scope of these accreditations. All other services will be referred to other professionals.

* Clients will fill out a medical history form and keep the therapist informed of any pertinent health changes including medications. If a client is deemed unsafe to massage for a medical reason, the client will need to refrain from massage until the condition becomes safe for massage to continue.

* All equipment is sanitized with a bleach solution. Clean linens/towels are provided for each client. The therapist will wash her hands or feet before and after each session.

* Clients are treated with fairness, respect, dignity and modesty at all times. Clients are asked to undress to their comfort level (naked is OK). The client will be properly draped with a sheet and towel so only the area being worked on is uncovered to insure modesty and privacy at all times. Breast and genital areas are never uncovered or worked on.

* Client will not come to a session under the influence of any drugs (including prescription painkillers) or alcohol.

* Clients are expected to adhere to reasonable cleanliness and hygiene.

* Massage Therapy For Health accepts cash or check for services. Payment is expected at the time of service. Receipts are available on request. Massage Therapy For Health does not collaborate with any health insurance or workman’s compensation agencies. A returned check will incur the current bank rate for "returned" checks.

* There will not be a confirmation call for appointments unless requested by the client in advance.

* Sessions begin and end at scheduled times. Sessions which start late due to client tardiness will end at the appointed time and will be charged at the full session price.

* Clients must provide 24 hours notice of cancellation. If less notice is given or a "no show" occurs, full payment for the missed appointment is expected. If Massage Therapy For Health must cancel a client’s appointment with less than 24 hours notice, the next appointment of the same duration will be free of charge. The exception to this is if the therapist is sick and working on a client would pose a health risk to the client.

* Clients are asked to not have children in attendance. There is no childcare available. If it is unavoidable to bring a child, the child will remain in the treatment room under the parent’s supervision during the session.

* Children under the age of 18 may receive massage. The child’s guardian must sign a consent form. A guardian is always welcome in the treatment room during the session. It is recommended that a guardian be present in the treatment room or waiting room during the session for any child under the age of 16.
